The Student Services Coordinator facilitates accommodations for students with disabilities or medical conditions to ensure equal access. If you have a disability or medical condition and think you will require some type of accommodation, please complete this form and return it, along with documentation of your disability to the Student Services Coordinator. The registration process includes a counseling appointment to determine appropriate accommodations. Students registered with Student Services may apply for accommodation letters to inform faculty of their assigned accommodations each semester.
